Note: Thank you for your willingness to accept and entrust 104 Corporation to provide you free talent recruitment services. Please carefully read the following terms and conditions, thank you for your cooperation.
一零四資訊科技股份有限公司(私立就業服務機構,以下簡稱一零四公司)謹向您告知以下事項:
104 Corporation (also known as Private Employment Services Institution under the Employment Service Act of R.O.C.) hereby inform you the following items:
一、蒐集特定目的
1. SPECIFIC PURPOSE OF COLLECTION THE PERSONAL INFORMATION
「人才仲介」之特定目的(就業服務法所稱就業服務業務)。
The specific purpose of collection personal information is "Talent Recruitment" (also known as Employment Services Businesses under the Employment Service Act of R.O.C.)

二、蒐集個資類別
2. CLASSIFICATION OF THE PERSONAL INFORMATION
依照您提供的履歷資料,一零四公司會蒐集並處理個人資料類別包含:識別類、特徵類、家庭情形、社會情況、教育、技術或其他專業、受僱情形、通訊地址電話及電子郵件信箱。
According to the resume provided by you, 104 Corporation will collect and process the classification of the personal information including: identification, characteristics, family status, social activity, education, skill or other professional, employment status, mailing address, telephone and email address.

三、利用對象
3. TARGET OF THE USE OF THE PERSONAL INFORMATION
您提供的履歷資料僅供一零四公司,基於「人才仲介」之特定目的而蒐集、處理、利用與國際傳輸。與一零四公司有契約關係之徵才企業或合作企業,以及其所引薦的徵才企業則會基於徵才目的而蒐集您的履歷資料。
The resume that you provide is only for 104 Corporation to collect, process, use and international transmission based on the specific purpose of the " Talent Recruitment ". Hiring companies that have contractual relationships with 104 Corporation, will also collect your personal information according to the purpose of talent recruitment.

四、利用期間、地區與對象
4. TIME PERIOD, AREA, TARGET OF THE USE OF THE PERSONAL INFORMATION
您提供的履歷資料僅供一零四公司於簽約委託之徵才企業或簽約合作企業所引薦的徵才企業所在地區做為「人才仲介」特定目的之用。一零四公司會善盡個人資料安全維護之責,並將您的履歷資料儲存於獵才資料庫系統中,永久保存至特定目的消失或您要求刪除之日止。
The resume provided by you is only used for the specific purpose of “Talent Recruitment” in the area where the hiring companies have contractual relationships with 104 Corporation are located. 104 Corporation will do its best effort to maintain your personal information in secure manner and to store your personal information in the database system for permanent storage until the specific purpose disappears or the date when you request deletion.

五、個資完整與詳實揭露
5. ENTIRETY AND DETAILED DISCLOSURE OF THE PERSONAL INFORMATION
您所提供的履歷資料應完整與詳實揭露,若未提供、提供不完整、有所隱匿或提供不實資料,可能會造成一零四公司獵才顧問或徵才企業無法評估您是否符合職務條件而影響推薦或就業機會。
The resume provided by you should be disclosed in detail. If it is not provided, incompletely provided, hidden or false information is provided, it may cause the consultants of 104 Corporation or hiring companies failing to evaluate whether you meet the jobs and affect recommendations or employment opportunities.

六、行使個人資料保護法第三條的當事人權利
6. RIGHTS OF YOU TO EXERCISE UNDER ARTICLE 3 OF THE PERSONAL INFORMATION PROTECTION ACT
您若對於所提供的履歷資料擬行使查詢、閱覽、製給複製本、更正、補充;請求停止蒐集、處理、利用或刪除之權利;或有緊急事故或申訴需連絡時,請由本人於上班時間內洽詢客服人員02-27523104分機6688辦理。
If you want to exercise any inquiry and request for a review of the personal information; any request to make duplications of the personal information; any request to supplement or correct the personal information; any request to discontinue collection, processing or use of personal information; and any request to delete the personal information, please contact our customer service staff at 02-27523104 extension 6688 during working hours.

七、推薦人的告知
7. NOTIFICATION OF THE REFEREE
當您提供推薦人的個人資料時,請務必確認已事先對該當事人進行告知,使其了解一零四公司將會因此而蒐集、處理與利用推薦人的個人資料,做為人事背景查核之用。
When you provide the referee’s personal contacts, please be sure to confirm that the referee has been informed in advance, and you should inform his/her that 104 Corporation will collect, process and use the referee’s personal contacts, as used for reference check.

八、獵才顧問媒合方式
8. MATCHING SOLUTIONS
一零四公司獵才顧問於接受徵才企業之職務委託後,會就確認之職務內容,與您的學經歷及職涯發展意向確認相符後,向委託徵才企業進行推薦,經委託徵才企業面試與錄用後,完成雙方議定勞動條件之聘僱程序。
After accepting the appointment from the hiring companies, the consultants of 104 Corporation will make recommendations to the hiring companies after the confirmation and verification of your resume and your intention of career development. If the hiring companies after interviewing you and deciding to employ you, 104 Corporation will assist you to negotiate an employment agreement by both parties.

九、媒合職務
9. RECOMMENDING JOBS
一零四公司獵才顧問於接受本委託書後,會於您的面試程序中,清楚說明委託之徵才企業與職務內容。
After accepting this Agreement, the consultants of 104 Corporation will clearly explain the hiring companies and job descriptions during your interview process.

本人已事先閱讀、瞭解及同意以上告知事項的內容,並同意針對獵才顧問所推薦的職務與徵才企業負有不對外洩漏的保密義務,另同意獵才顧問得針對本人所提供的推薦人或前雇主的主管、平行單位同事或部屬進行人事背景查核作業,以及同意前述各該受訪者得以任何形式揭露或回覆予獵才顧問,供其製作人事背景查核報告予徵才企業做為徵才評估之用。
I have read, understood and agreed to the above mentioned items in advance, and I agree to keep the hiring companies and jobs recommended in confidential by the consultants of 104 Corporation. I also agree and authorize the consultants of 104 Corporation can conduct reference check with the referee, the supervisor of my former employer, parallel colleagues or subordinates provided by me, and agree that the above mentioned respondents may disclose or reply to the consultants in any form for the purpose of producing reference check report to assist the hiring companies for evaluation use.
